New Patient Registration

Our practice list is open and we are now taking on new patients.  For any non English speaking residence we offer an interpretation service please ask for this service when booking your appointments.

Please call in for an application form. Please bring in your completed and signed medical cards for each person requesting to join the practice. If you don't have these we do have another form for you to complete.

You will be asked for proof of identity; this will be one photo ID and a utility bill with your name on for the address you are registering from.

You will be invited to attend for a new patient medical and will not be fully registered with the practice until you have had this medical. We will request your records from the Health Authority once you have had your medical.

The practice will only refuse an application if you reside outside the practice area (pictured opposite) or if the list is closed.
